Maintaining the tradition of community healthcare at home is what Breckinridge Memorial Home Health Agency strives to achieve.  We are a home owned and community based agency licensed by the state of Kentucky, with Joint Commission Accreditation, and certified by Medicare to provide home health services in Breckinridge, Meade, and Hancock counties.

Services currently available through our agency include Skilled Nursing, Physical Therapy, Speech Therapy, Aides services and Community Based Waiver.  Individuals may benefit from home health services if they are in need of IV therapy, injection teaching, wound care, diabetic instruction, urinary catheter care or rehabilitative services.  Also patients being discharged from the hospital needing follow up assessments may qualify for home health.

A physician, patient, family member, health care facility or community agency may make request for home health services.  However, any services must be approved and ordered by a physician and considered reasonable and medically necessary for the treatment of an illness or injury.  Information regarding these services can be obtained by contacting our office Monday through Friday, 8:00 – 4:00 CST, at 270-756-2662 or1-877-850-8725.

Personal Response and Support Services

Home Health also provides Lifeline Service. Lifeline gives you the confidence and freedom to live on your own. With Lifeline there is always someone there to send the help you need, 24 hours a day, seven days a week.

With Lifeline your loved ones can rest assured that you are safe, secure and happy in your own home. With just a push of your personal help button a highly trained Personal Responce Associate at Lifeline Monitoring Service will assess your situation and send the help you need. It may mean sending an ambulance or calling a family member or neighbor to look in on you.
